Child Protective Orders

An adult uses these forms to ask the juvenile court for a protective order for minor children less than 18 years old, when protection for those children is not requested as part of an adult's request for a protective order. Minors who are at least 16 years old can also apply on their own for a protective order using the forms for the adult process found on the Protective Order Forms page.

To get court-ordered protection using these forms, fill them out and file them at your local juvenile court. See the court's directory for contact information for all juvenile courts.
